Accurate iFR Measurement Requires 20 Seconds After Saline Injection to Targeted Coronary Artery.

Background

Accurate measurement of wire-based physiological indices requires to replace guiding catheter from contrast media to saline. If we start iFR measurement immediately after saline and/or contrast media injection, the value of iFR will be lower. However, there is limited data about howlong we should wait after saline injection to the coronary artery.

Methods

We collected data about twelve patients with stable angina pectoris in left anterior descending artery (LAD) lesion who underwent iFR at our hospital from January to April in 2019. We used Volcano S5 (Phillips) and Verrata Plus wire(Phillips). Taking into account of the volume in the catheter, we injected 6 ml of saline during 10 seconds before iFR measurement. After saline injection, we measured the time to obtain the stabilized iFR value, which was defined as cognitive same value showing during at least 2 heartbeats. The cut-off iFR value was 0.89. We also collected patients¡¯ clinical data from our catheter laboratory data.

Results

The mean age of the patients was 71 ¡¾ 13.64 years(83.3% male). Three patients had old myocardial infarction in the LAD lesion. Six patients had diabetes mellitus.The time to stabilize iFR value after saline injection was 17.3 (7.53-19.75) seconds.  The time after saline injection in patients with positive iFR value was longer than in patients with negative iFR value [18.7 (13.15-21.15) seconds vs 6.15 (5.05-15.05) seconds, p=0.02 ].

Conclusion

To accurate the measurement of iFR, we should wait about 20 seconds after saline injection to coronary artery. Patients with positive iFR value requires more waiting time than patients with negative iFR value.
