Patient came with chest discomfort and uncontrolled diabetes, and past history of PCI (2013,2015,2018,2021). We did PCI and put stent on LAD. After 2 months, he came again with the same symptoms, then we performed angiography.

Procedural Step
The patient comes to the hospital with complaints of chest discomfort and uncontrolled blood sugar. Previously, the patient had stent (PCI) installation in 2013, 2015, 2018 and 2021. Currently, the patient came back after about two months of installing a stent (PCI) in the mid LAD with the same complaints, namely chest discomfort and high sugar levels. We did angiography and we got a stent fracture in the mid LAD, then we did an IVUS examination to confirm the morning of the stent fracture and determine the type of fracture. After knowing the exact location of the stent fracture, we did the preparation using a Cutting Balloon and we put DES in the mid LAD (the place where the fracture was).
Case Summary
Stent fracture can occur at any time; it is most common in RCA. However, in this case, it occurred in LAD. And when using Imaging (IVUS) we can more definitely see and confirm the lesions or fractures that occur.
