Using Pedometer to Assess Physical Activity Outcome in Ambulatory Severe Mitral Regurgitation Patient After Cardiac Intervention (The Pilot Study)

Background:
According to 2017 ESC/EACTS and ACC/AHA valvular heart disease guideline, the transcatheter (MitraClip) approach to treat mitral regurgitation in the high risk or prohibitive risk patients was recommended. The quality of life and functional class were improved after the procedure. However, the results came from questionnaire which might be inaccurate due to patient¡¯s subjection. For useful accurate tool, we test the ability of pedometer to evaluate the physical (walking) activity in severe mitral regurgitation patients undergoing transcatheter treatment.
Methods:
Ambulatory patients with symptomatic severe mitral regurgitation who undergoing MitraClip therapy in Ramathibodi hospital were studied. The step counter by pedometer was recorded 1-week before and during a similar time period toward the 4-week after transcatheter treatment. Patients also completed the six-minute walk distance (6MWD) and 36-item Short Form Survey (SF-36) questionnaire at the beginning and end of study. Paired t-test were used to compare walking steps, 6MWD, plasma NT-pro BNP level, SF-36 scoring and echocardiographic parameters. A regression analysis was used to study the trend of walking step by pedometer after transcatheter treatment.
Results:
From January to April 2019, 5 patients were met the inclusion criteria. All of them could follow the study protocol. After MitraClip therapy, there was no statistically significant differences of 6MWD and plasma NT-pro BNP level (p = 0.24 and 0.12, respectively) but the physical activity by pedometer tended to be improved (p value for walking step by pedometer = 0.07) at 4th week after MitraClip. There was significantly improvement of quality of life by SF-36 scoring system from baseline (mean SF-36 score change = 28.0 ¡¾ 17.8; p = 0.02). The step counter by pedometer showed quickly ambulation of the patients in a few days after procedure and improvement tendency of step counter at 4th week after transcatheter treatment. No serious adverse outcomes were reported during and after the procedure.

Conclusion:
The physical activity (walking step) by pedometer, 6MWD and plasma NT-pro BNP level after MitraClip had no statistically significant differences from baseline, but the step counter by pedometer had shown the improvement tendency at 4th week after MitraClip and quick recovery of the patients.
