Investigation of Invasiveness in IVUS Guide EVAR - Contrast Media Usage and Exposure Dose | |

Background:
EVAR is becoming the standard treatment forAAA, and relatively good results have been obtained even in high-risk cases. Itis a procedure that requires the use of radiation and contrast agents, and itis important from the point of view of complications to pursue how these uses can be reduced.

Methods:
There were 130 cases of EVAR for abdominal aortic aneurysm and iliac aneurysm in our hospital from July 2011 to March 2018, of which CIA was used as a landing zone, and coil plug embolization and stent placement were performed. There were 36 cases not included.

Results:
IVUS use was in 15 cases. The amount of contrast medium used was significantly lower in the IVUS group and non-IVUS group (99.0 ¡¾ 59.2 ml vs. 164.8 ¡¾ 53.4 ml p <0.01). In terms of exposure dose, the IVUS group had low exposure (667.7 ¡¾ 352.4 mGy vs. 1615.2 ¡¾ 949.8 mGyp <0.01), and the procedure time from puncture to the start of closure was short (103.0 ¡¾ 27.7 min vs. 143.7 ¡¾ 45.2min p <0.01).

Conclusion:
With IVUS, it is possible to reduce thec ontrast agent and reduce the exposure dose, and it is expected that EVAR will increase for high-risk patients such as patients with renal impairment and the elderly. I think it is necessary to pursue a reduction in exposure dose.
