The Impact of Revascularization on Left Ventricular Global Longitudinal Strain - A Comparative Study Between ACS and CCS Patients

Background:
Primary percutaneous coronary intervention (PCI) is a goal standard reperfusion strategy in the setting of acute ST elevation myocardial infarction (STEMI). The aim of this study is to establish the PCI to ACS patients during index hospitalization is superior than delayed PCI.
Methods:
This prospective observational study was conducted in NICVD included 92 patients which was divided into ACS group and CCS group and Underwent PCI. Patients with heart failure, cardiogenic shock and getting fibrinolytic was excluded from this study. The primary outcome after angioplasty was assessed by 2D global longitudinal strain (GLS) using speckle tracking echocardiography (STE) and secondary outcome was also determined
Results:
The mean age of the study population was 51 years with male predominant. There is no significant difference between two groups in respect of DM, HTN, Smoking, Dyslipidemia and family history of premature CAD. LV GLS after angioplasty was much lower in ACS than CCS group of patients. In terms of secondary outcome there was no significant difference between these two groups.
Conclusion:
The establishment of epicardial coronary arterial flow is not all to recovery of myocardial functions. Early angioplasty during index hospitalization of ACS compared to CCS patients was superior than delayed angioplasty.
